If you choose Taylor Made Outcomes as your Support Coordinator, we will work alongside you to help you access a mix of supports that meet your needs, build your capacity, maintain relationships, live more independently, and participate in your community.
Your Support Coordinator can also assist you to make the most of your funded supports and help you prepare for future plan reviews.

Taylor Made Outcomes claims from your NDIS plan for the time spent assisting and supporting you. Support Coordination is generally claimed in line with the relevant NDIS pricing arrangements and based on the level of support provided.
