Principles of social justice, human rights, collective responsibility, and respect for diversities are central to social work.

Taylor Made Outcomes Social Work through the NDIS
Social Workers at Taylor Made Outcomes will support you to identify your individualised goals and engage with you to create a plan on how to address any barriers you may be facing in achieving these goals.
We pride ourselves in trauma-informed, strengths-based and person-centred approaches to ensure that you remain the core focus. This can assist to increase your capacity to maintain relationships, live more independently and be included in your community.

We will meet with you and complete an initial appointment to identify your needs and goals. With your consent, information will also be gathered from the important people in your life.
Following these discussions, you will have access to a holistic report developed by your Social Worker, that will highlight strategies to assist you in meeting the goals you identified.
Additionally, we can assist you to:
- Understand your disability circumstances and environment
- Discuss with you about what is meaningful to you and help you to create ways of achieving your goals
- Assist you to build skills in areas that are important to you
- Offer you coping strategies, mindfulness, and relaxation techniques
- Help you to understand the relationships in your life and implement strategies to maintain and improve them
Costs
Social work is funded in your capacity building budget Improved Daily Living. Taylor Made Outcomes charge your NDIS plan for the time spent on assisting and supporting you.
